I am going to apply for Visit visa to USA. I am from India and interested to visit Denver city.

What is the step by step and quick process..?

Where to apply,
Whether I need to submit documents at VFS

First I have to pay fees at bank, or I have to pay fees and then apply.

This about the best I can do. For your flights you need to do a lot of research on-line. I not sure of directions the Air Lines take, but, assume Los Angeles, California (West Coast) or New York on the East coast be your points of entry. IF, you can find a US based Airline that fly to India it may be cheaper than Air India. It just something you need to research on-line.

First you have to see if you can get a support letter from a family member in U.S.A. This way the immrgration will know that when you come here in U.S.A. you will have a place to stay. Pull a application form (visits form) from the wed site and fill it out, now a days most of the imformation, you can get it from the internet. If you are married then you will have to get a permit from your husband to travel. About the fees, different country have different fees and different rules. You cant fix a date because first you need to know if your application is accepted or not, however you can state your date you want to visit on your application. Again for the time you will have to go to Map quest to see how much time it will take or you can find it out from local travel agent. Buy dirrect from the travel lines and also try to shop on the internet. I hope This helps.
