well, you could say something like this... "gee, i'm flattered, and i think you're sweet, but i'm in a committed relationship, and therefore not available. i really wish you well, and hope that you find the right person for you."
it's more than correct to inform her of your status, it's mandatory. and, remember, ex's are ex's for a reason. if you guys broke up once, why should you think that things would be any different now.

I think you should be honest with her and tell her the truth. It's not fair to your current girlfriend for you to be going out with your ex. You are in a relationship and your ex should respect that.
