Psychopaths have a physiological problem in that their parts of their brains are not as sensitive to the chemicals produced in their brains as a regular person. Therefore the amount of a stimulus required to produce the same reaction is much larger, for example what a regular person feels watching a scary film would have very little affect on a psychopath and they would have to actually go out and be in real danger to feel the same effect. Hence the extreme behavior displayed by psychopaths.
According to research they also seem the lack the ability to form the 'if I do this punishment will follow' idea.

What is sanity?
Who determines what sanity or insanity is?
As beings we are all physiologically so very different. Just because one person, acts or reacts differently, compared to the majority, doesn't necessarily mean that they are insane.
They just see life in a different way.
