i live in rancho Cucamonga, CA but LA is only an hour away so were can i get one. how much does one cost, what type of grades do i have t have, any restrictions????

Um, do you mean an Entertainment Work Permit? You fill out the form and mail it in to the San Bernardino office. You need to have school district officials sign and stamp the form so mom or dad will need to take you and your most recent report card to the district office to get the school district's official seal stamped on the form.

Wow, I think Marianne D has it covered, except your question about grade. You have to have good enough grades (probably a C average or better) and satisfactory attendance or the school district may deny your request. The permit is free, just pay the postage.
