My teacher gives us a good week heads up. So what I do is ask her what the important parts to study are and she actually makes us study guide. Maybe you could ask your teacher to make a study guide or at least tell you what to study and make your own study guide. The study guides are really helpfull mostly everyone in my class gets As and you can see it because all of it is highlited.

You may have a genuine testing disorder. The special education center at your school should be able to determine if you do, and make arrangements to lower the stress for you during exams.

I hate to say this, because my husband was a professor at two large universities, but sometimes your mind wraps around a problem differently than your instructor's, but you land in the same place. And sometimes, your instructor isn't flexible enough to figure out how you arrived at your (right) answer.

If this is only one class, I'd shake it off. Sometimes there are teachers whose teaching style just don't mesh with your learning style. and that's okay, so long as it's not most teachers. If that's the case, the special education department should be able to find what your learning style is, and make you more comfortable while learning, whether it means special tutoring, or asking the prof to go slower, or to repeat certain parts, or to provide you with a copy of the notes for the day's lessons so you don't spend your whole time copying notes.

Talk to your counselors. It's their job to help you get a great education, and believe me, they will enjoy helping you.
