The odds on favorite is Regulus Black but JKR has confirmed or denied nothing. She did say when asked if R.A.B. was Regulus Black that that would be "a fine guess."
There are also theories that it could be Mr. Borgin or Ludo Bagman's father, and have actually managed to make a good case. Though, the locket found in Grimmauld place is a too much of a coincidence, in my opinion, though it is certainly possible that it is another Black relalative (suggests have been made that it may be Sirius's grandfather who we know nothing about other than he may have donated a large sum of money to the Ministry).

It's most likely Regulus Black. In all the translations into different languages, the names change too - the R.A.B. initials are ALWAYS changed to match Regulus' initials, even when they change through translation.

Sirius' brother, Regulus Black. There ain't no other way about it. I don't think anyone else in the book has those initials

Well, no one will know for sure until book 7, but my guess is Sirius' brother.
