All you really need is the other person's information.
If there is a question of fault, a police report is helpful, but not necessary.

Only if you want to get paid. Insurance companies do not normally re-imberse you without complete documentation.

No, but I strongly recommend that you get a police report if you believe that its not your fault. If you think it is your fault try to avoid it.
