You can go to the county you were born in (this is faster), or you can go directly to the state. You will have to have proof of who you are in order to get a certified long form. Two forms of ID will be needed. This is to prevent identity theft. It can cost anywhere from $10-$20 depending on the agency you contact. Well in my state you get them from teh State Department of Health. I would imagine NY is the same. There is generally a nominal fee. ( I had to pay like 12 dollars for mine about 6 years ago.
