Julio Franco of the Mets. He still plays, he just chooses not to wear one.
Tim Raines was the last player to wear one, in 2002 - for the Florida Marlins.
The cut-off point where people wear grandfathered was 1983. Franco started playing in April 23, 1982.

Shawn Green because his ears won't fit in a regular one.
Barry Bonds just wears his head because his brain is too big to fit in a helmet.

I know Carl Yaztremski was the last guy who did not have to wear a helmet at all, but am not sure about your question.

I'm pretty sure it was Gary Gaetti, but it may have been Tim Raines. I recall the cutoff was 1980, so Franco would just have missed it.
