When the pitcher puts a guy on base via a walk, hit or hit by pitch then that guy scores. Even if a pitcher gives up a single and a new pitcher is brought in and then that pitcher allows the single to score, that run is charged to the previous pitcher for allowing him to get on base. The basic definition would be a pitcher allowing a batter to get on base and score.

Simply put, it's a run for which a pitcher is held accountable.

Any run that scores due to an error on the part of the defense (including the pitcher) would not be an earned run. In some cases, the scorer will have to reconstruct the inning to see what runs would or would not have scored without the error(s).

In all cases, they're supposed to give the benefit of the doubt whenever needed to the pitcher.
