Having ample prenatal care, taking prenatal nutrients, treating STDs and keeping off medications and alcohol might decrease the chance of getting a baby with a bodily deformity, however a few are simply flukes of nature and in spite of the dad and mom doing the whole thing proper a little one can correctly be born with a deformity.

The baby, or fetus, grows in the uterus of a woman. After 40 weeks the fetus is (typically) developed enough to be born, it passes through the birth canal, or vagina, and there you have it, a new baby...with a lot of work from mom of course to get the baby out.

Well I had heard the cabbage patch but according the intarwebs after an ovum is fertilized by sperm the baby grows in the uterus and is birthed through the vagina.
