There are at least four ways of doing it, four ways of answering this question.
1. Combination of two existing finishers:
Example:
a) The Suffering: A Batista Bomb followed up by a Wall of Jericho. After smashing the oppponent on the mat the superstar does not let go of the opponent's leg but immediately delivers a wall of Jericho
b) The Revolution (created by Legendkiller PAC, one contributor in the wrestling section). An F-5, wherein the spin is halted in mid-air and an RKO follow suits

2. A variation of an existing finisher
example:
Twisting Spear: Supertar performs the usual spear but twists in mid-air, making him in the lying position as he hits opponent's abdomen.

3. Inspired by a movie or video game move
Greacian Spear- Think of Umaga's Samoan Spike and Achilles signature move in the movie "Troy". Can you dig it?

4. Tag Team Finisher
Hell Drop - Kane raises up an opponent with a chokeslam. while opponent is up there, Orton delivers an RKO
