If I was the NL manager my pitcher would have already pitched the Top of the 1st - so yeah, pinch hitter time.
The AL Manager has a tougher decison. You don't say how many outs so I guess if there's 2 outs he stays in, one out, not sure and no outs, he's history.

Not a chance. If he is up then we have scored three runs already. If he makes the third out, we start the second inning with the lead off hitter. However, if it is the bottom of the 1st and we were down by more than five because the starting pitcher had given up a bunch and we had not replaced him for some strange reason, then I suppose I might pinch hit for him at that point.

Hmm, well if the bases were loaded in the first inning, and the pitcher was up.
#1 - Hit
#2 - Hit
#3 - Hit
#4 - Out
#5 - Out
#6 - Hit, 1-0
#7 - Hit, 2-0
#8 - Hit, 3-0
So it's 3-0 when the pitcher comes up to bat. Don't take him out of the game just because you don't want him to hit, no matter how plentiful pitching is. If this was the top of the inning, and he was taken out, he wouldn't even get to pitch! 3-0 is a nice lead, keep him in.

Defnitely not if I was the AL because that would mean Haren hadn't even pitched and he deserves the chance to pitch in front of hometown (sort of since Oakland is across the bay) fans. Plus, at a minimum you have already scored three runs because it can't work any other way mathematically.
If you are the NL, maybe but doubtful. Peavy would have already given you one inning, though you probably want him to go two. Unless he threw an inordinate amount of pitches in the first inning, I let him bat.

If it was the top of the first inning I don't think the manager had an option. I believe all star rules dictate that the named starting pitcher has to throw a pitch.

If it is the first inning and the bases are loaded and the pitcher is up you've already scored at least 3 runs. I'd let the pitcher hit. What the heck.

Depends on the number of outs but if I'm the AL manager I just might. Given this is chance to blow the game wide open and the starters never pitch more than a couple of innings anyway.

No, because if you already got through the lineup in the first inning than the very least your team could have scored would be 3. So, if you had 3, 4, or 5 points already, there would be no real reason to stress.

No way. It's the All-Star game. In the last three or four innings, then you can do things to try to win. But for the first five or six let the guys play, and let them enjoy the all-star game that they earned.
