the story of ricky.. ricky-o was the most violent thing I've seen, but that is live action manga
anime-wise I would say..urotsukidoji:legend of the overfiend ..definately over 18's only

Although its only a movie and probably isn't as gruesome as others, 'kite'? Its probably just the most violent one I know.

I don't know about "ever", but Fist of The North Star was pretty gruesome for it's time.
