It's determined by MLB. It usually switches between an American League or National League park, but sometimes it's not the case, last year's game was in Pittsburgh and this year's is in San Fransisco, both NL cities.
MLB usually awards the location using a very subjective guide, but the requirements are usually that it has been a long time for the city to host the game, and to showcase a new ballpark.
MLB usually only awards the sites about two years ahead. Currently, after San Fransisco, the game will be held in New York (Yankees, to say goodbye to Yankee Stadium) in 2008, and St. Louis in 2009. While a specific year has not been set, the All-Star game will be held in Kansas City at Kauffman Stadium within the next few years, perhaps as early as 2010. Bud Selig has promised the Royals will host the game between 2010-2014 in exchange for the voters approving 150+ million dollars in renovations to the stadium. Kansas City is the only confirmed future host.
Cleveland hosted the game in 1997, so it may be sometime before the game returns. I would expect Cincinnati would host the game very soon to show off their new stadium, possibly in 2011, 2013, or 2015.

The commissioner's office makes the selection, usually with a two-year lead time. So the 2010 site will probably be announced early next year.
Upcoming:
2008 -- Yankee Stadium, NYC
2009 -- Busch Stadium, St. Louis
Cleveland had the ASG in 1997, and isn't likely to come around in the rotation any time soon, a decade at the very least.
Cincinnati hosted in 1988, but has a new park (opened 2003) so MLB will probably want to show it off soon. Maybe 2011, 2013, or 2015; there's a few other new NL parks that haven't yet hosted either.

It is a combination of rotation, and team lobbying. There used to be a strict rotation of ballparks by league, however this is the 2nd consecutive year for National League park to be used. Often the last season of a legendary ballpark, or after a new ballpark is built, will a park get selected.
Since Yankee Stadium, in it's final year will be used in 2008, the next season it is up for grabs. Looking at patterns the likely All-Star homes will be, if going back to a National League park, would be St. Louis 2009, St. Peterburgh, FL 2010, and Cincinnati 2011. If the Fremont (for Oakland), and Minneapolis(for Minnesota) get completed by 2010, both will be in the All-Star rotation soon.
If it stays in the American League for 2009 I see the rotation as Anaheim 2009, St. Louis 2010, Minneapolis 2011, Cincinnati 2012.

well it goes back and forth between an nl park and an al park. the mlb people vote as to where it will be held.
