I'm guessing that yes, the 200-mile-high club already has members. It's only a matter of time before NASA sends up a husband/wife team or something like that to investigate the effects of being isolated in space on all aspects of a marriage. If we were to ever colonize other planets, you'd need to know about that.
If NASA doesn't do it, it will definitely happen when space tourism becomes common. There'll probably be a line of couples at the lavatories during the zero-g portion of those short space-shots.

Officially... no. But I've heard rumors.
I think the um... 'rendezvous & docking' procedure would be pretty easy... but maintaining the 'hard dock' would be easier with gravity.

I would venture to say no because they have more important things to do like fly the shuttle, but those cameras aren't on 24-7.
