She would not automatically be vice president. As with Nixon and the resignation of Agnew a vice president could be appointed by Congress to serve out the rest of the term. Otherwise there would be no vice president. However if Bush were unable to be president and Cheney became president under the Constitution without a vice president if something were then to happen to Cheney she, Pelosi, would be president.

No, because impeachment merely means being charged with offenses in the House of Representatives. Bush would have to be convicted by the Senate to be removed from office.
Mere impeachment is nothing more than a blemish on one's record and has no real meaning.
