I want to change my last name back to my birth last name. My step-dad adopted me and I have his last name right now, but I want my old last name back and I have my birth certificate and old SS cards to prove what my original last name was.
Does this cost a lot of money, do I need a lawyer, etc?
I'm an adult and not a minor...but my step-dad's last name stuck with me...

I live in Naperville/Aurora, Illinois. Sorry for sounding so clueless, but what is this deed poll and other stuff that you guys are talking about?
Can anyone guide me as to what the right place is to go? Websites say the name change will cost 100 bucks or so..that's not too much for me.

Can I get these affidavit/name change forms from a courthouse? I looked for the ones online and it's telling me to pay 60 bucks for a name change kit and download it online.

You will have to have an Deed Poll and an Solicitor can do that, you will have to pay.

You must file a petition with your local county court requesting a name change. You can buy the papers at one of those online legal services or just ask at the courthouse-they probably have the forms there. You do not need a lawyer. When you initally present the forms to the Prothonatory of the Court you may also have to submit fingerprints and other documentation for a background search. Then they will send you a letter telling you that you are clear, legally and you may file. You then go in front of the motions clerk and get a date for your hearing. Typically you then have to advertise in two papers regarding your name change so anyone who may object has the opporutnity to do so. My name change ended up costing about $350 in various fees. I did not use a lawyer. It was a bit cumbersome but not impossible. Additionally I paid about $250 for the paperwork from one of those online legal document companies but when I went to to court to request the change they gave me a packet I could have used instead.

You will need to fill out an Affidavit saying that you want to change your name lawfully and not because of any Felonies or debts owed - but; because of the circumstances you stated above.
Get it Notarized for free in the court house of your choice - THEN ...
Go to the District Clerk's Office in your County - and request the name change form - and then have it filed in a District Court. A District Court carries the most weight and all will accept this over any Deed or probate court or county court or even the justice of the peace.
It is correct that it will cost around $150.00, but; it is legitimate and you will have lawful right to your claim once signed by the judge.
I will list a link below that if you read it - you will see how a Whole City has persecuted me with Religious Hate Crimes as they admit fully of criminal activities they did against me - deprivation of public Information/Open Records Act ... and you will see that they acknowledge my name change done as I told you above ... and persecute me over it.
I forgive them ... and am kicking their butts in court right now ... just me fighting as Sui Juris ... and they got 10 Plaintiffs against me - most of the Plaintiffs are the City Attorneys, most the rest are employees of that legal office ... Assistant City Attorney already resigned and fled ... she will be brought back soon to answer for her crimes - I do fully forgive her though - but; it is like slapping your thumb with a hammer ... you can say you are sorry for doing that to yourself - but; it won't make the pain any easier ...

Yes definitely you can change your last name to that old name you have before. Every citizen is given this privilege.
Yes also to your question does it cost money. Absolutely it does cost you some I should say, and also you need a lawyer to represent you in the court of law. You can confirm first to your choice of lawyer in town, this is a civil case so therefore you need a lawyer that is familiar with the civil code. He will give you a brief background of what you should prepare(documents..etc)to present in court. You will definitely pay his services(attorney's fee, docket fee...etc). Good luck.

You just go to the courthouse and ask for the name change forms, fill them out and have them notarized. I think it does go in front of the judge for final signature and you may even have to appear in court for it with a reason why. Also there is a cost and I hear it is not cheap.

All you need to do it get a Change of Name Deed and get it notarised. Not sure of costs, but its not overly expensive.
Then you simply start using that name! For things like bank accounts, merely present the Deed and request they change your account name.

You will have to go to court to do this. I believe you can get papers to file for a name change from a few places ,maybe even the Internet.

contact the probate court where you live. here it costs about $150. you can do it without a lawyer.
