Generally when you name a star after someone, you name it after them... ie with their name.
I wouldn't waste money on a star naming thing though, it's not official and is purely for entertainment only. You could print up your own certificate naming any star you like after your grandmother for free (well the cost of the paper I guess).

good luck with the star thing, all the visable ones are registered and taken, you'll need a super powerful telescope just to see the thing you wish to name.
anyways, if it was for your grandmother, then the general rule is to name it after her. but what would she like? did she have a cherished cat? or a favorite song? what would make her happy and at peace.

You could use her first, middle, or last name. I think the whole star naming thing is a great idea.
