I assume you are in the UK. When I go to The States I take a little cash in dollars and buy some US dollar traveller's cheques before I go - in the US there is no interest in any currency other than their own so pounds sterling are no good! I believe you can get traveller's cheques from post offices as well as banks. - You can use them like cash in the US. They have obvious advantages - you don't have to worry about having loads of cash about your person; if you lose the cheques they can be replaced; places in the US (shops, restaurant, bars etc.) take them as payment and give change in dollar bills or you can change them in hotels.
Maybe you already know this and I have missed the point of your question, but a combination of a credit card and using dollar cheques can cut out the need to exchange money when you get there as you can spend them just like cash.

If you are a student and have an ISIC card go to your nearest Travel Ex and take a wad of pounds and exchange for dollars. The rate is so good right now, and with no commission charge, you'll get the best deal through Travel Ex. If you're not a student then it really doesn't matter, unless you're exchangng thousands of pounds into dollars (which wouldn't make much sense because arriving into the USA they make you claim over a certain amount if you are. Looks sketchy if you are taking a ridiculous amount of cash).
Also if you have a Citibank or HSBC account there are loads of Citibanks and HSBC cash points in the states, and you won't receive service charge (if you do it's no more than a dollar), so you can just withdraw then, but you might have to deal with the exchange rate your bank decides to be suitable for that day (including commission charge). So I'd take a couple hundred pounds, exchange it, and if you need more money then withdraw it.

go into your local travel agents that has a foreign exchange desk and they will do it there and then , that's what i did when i went to Florida, or you could go to the bank, but check around though so you get the best exchange rate you can find.

My wife gores to M & S Get American Express travellers cheques as well. Theytare excepted as cash in many shops and are more secure in the case of theft

Depends on how much you want. I found American Express gave good rates. Also try M&S or your post office. Never wait till the airport to change. Their rates are worst.
