I have a US Passport and a round trip ticket, showing a 10 month duration. Do I get a Visa or extension immediately at the Manila airport, or must I go to a Consulate office, before my 21 days is expired? Must I repeatedly get short extensions, or can I just get the 10 months all at once?

No you can't get them all at once, you get a 21 day visa when you enter and then you can extend for another 38 days and then every 59 days up to 1 year, now if you are here for more then 6 months you will need to take 3 passport pictures and get a exit visa and clearance before you can leave which is free, here are the fees and the place you go to get the ext. http://immigration.gov.ph/cms/index.php?option=com_content&task=view&id=118&Itemid=43

Visas require 1 application form, 1 photo, onward/return ticket, proof of sufficient funds and $30 fee for single entry and $60 or $90 fee for multiple entries. Work it out with your embassy. Upon arriving at the airport, you are normally given a social pass for two weeks. If you have worked it out with the Philippine embassy from your country that you stay for six months, then better. Before your required stay expires, be sure to work out the extension at your consulate. The length of extension depends on the level of ties your country has with the Philippines and the existing policies between them.

You can do it later few days before or after your visa will expire. then you will be asked to pay for an extension depends on how long.

Extension of Authorized Period of Stay in the Philippines

In case of a temporary visitor’s visa holder whose stay in the Philippines will exceed the authorized period of stay, he/she will have to report to the Bureau of Immigration (at any port of entry in the Philippines), secure an extension of stay and pay the corresponding immigration fees.
