It means that you are about to accept responsibility for your mistakes and sacrifice yourself for a cause. For example, a CEO of a company that makes many poor decisions may resign or "fall on his sword" rather than be forced out by the Board of Directors. Oftentimes, the end result is inevitable but it is the way in which the person responsible exits. Falling on one's sword saves face most of the time.

In Roman times if a general lost a battle it was deemed unacceptable to the emperor. In order to redeem himself and preserve the honour of his family and, incidentally, to save them from being put to death he was expected to sacrifice himself by committing suicide. This was done by kneeling down with the sword pointed at the midriff then falling upon it. Very similar to the practice of hari-kiri performed by shamed Japanese officers in WWII.
