My Understanding of the "proper" format may be a little off but I think it goes like this for formal invitations:
A Married woman is Mrs John Smith
A Widow is Mrs. Anna Smith

Just in the sense that instead of Mrs. So and So it become Ms. So and So---but nobody really seems to follow the format anymore. Grammar seems to be playing a less important role, probably with the availability of text messaging. I wouldn't fret about it.

If the first wife was married to him before he died her name would remain the same unless she got it changed in the divorce or if she got remarried .
If the second wife was married to him when he died, her name would remain the same.
Death does not automatically change your last name, however you can petition the court for a name change if you want.

no. when a woman is widowed she is still mrs. ___ unless she gets re married or decides to go back to her maiden name.
