I don't know but they are related. The Black's(Narcissa's family) and the Malfoys are closey related!

Cause Lucius is just one step away from being a drag queen.

Not my joke and not actually meant to be mean. It's actually based on an interview Jason Issacs did with the San Fransico Chronicle September 3rd of last year. Issacs' may well have been joking, or maybe not, but it was worth a bit of a chuckle.

On a more serious note, they may look so much alike due to how few pure blood families there are. The pure bloods marry within their own families. Not their immediate families, but rather their marrying cousins, second cousins, third, etc. Most of the wizarding world is half-blood and muggle born. That allows a lot of diversity within most of the wizarding word. For the pure bloods, with far less diversity their bound to start looking more and more like each other. Obviously it's still a ways away as the Weasley's are pure bloods, but just how far off is the question.

If you mean in the movie, that's probably because the director auditioned for a woman who looks like Lucius and also with the help of makeup,. After all, in the story, the Malfoys marry their own cousins to remain pure (*as in the blood)

They are probably closely related. There aren't too many purebloods left in the wizarding world, so they end having to marry there distant relatives. I doubt that they are cousins though, I think you are confusing them with the Gaunts, who married there cousins and made them all have birth defects.

They are about 2 cousins

They are married. The illustrater probably made them appear similar to show that they go together, like socks that are a pair look alike.

All the pure-bloods are interrelated. As Sirius says in OotP, "If you're only going to let your sons and daughters marry pure-bloods your choice is very limited; there are hardly any of us left."

They are, however distantly, related, as they are both pureblood wizards. I doubt J.K. Rowling meant to have them look alike as anything more than a hinting at that.

I guess they look alike because they probably have the same expression on their faces and they are both blonde and blue-eyed. But if you mean in the movies... it doesn't really mean anything.
