Carl William Mays (November 12, 1891 - April 4, 1971) was one of the better right-handed pitchers in Major League Baseball from 1916-1926, but he is best remembered for throwing the pitch that struck Ray Chapman in the head on August 16, 1920, making Chapman the only major leaguer in history to die as a direct result of an on-field incident (he died the next day at a NYC hospital).
Been a couple football players die but this is the only baseball I know of.

Yes, there has been death as a result of a baseball game. The only player to have ever die as a result of a baseball game was Ray Chapman who, on August 16th, 1920, was hit in the head by a baseball thrown by pitcher Carl Mays. He died in a hospital later. His death was a reason why the spitball, the pitch allegedly thrown by Mays, was eventually outlawed.

Darryl Kile is NOT the last active player to die. Just a few months ago, Josh Hancock of the Cardinals was killed in a car accident. Also in October Cory Lidle was killed when his small plane crashed into a building in New York City.
But to answer the original question, yes, Ray Chapman was hit in the head by a pitch in a game and was killed.

Yes, one time. A player broke his neck once in a game.
Some argue that Tony Conigliano of the Boston Red Sox died as a result of a bean-ball he took. He actually died several years later, but it destroyed his career.

Ray Chapman died in the early 1900's. This was before helmets were invented. And he was beaned right in the temple of his head at the plate.
