Ok it sounds pretty important so i will tell you how to get it. You first have to phone 1-800-242-2742 quoting reference number 124-2334, the lady may pretend she doe'snt know what you are talking about this is a ruse, give her the code word "shazzam" you must yell it very loudly. It may take you yelling shazzam over and over to get her to give you the e-mail address but i assure you persistence will pay off.
