The batter can switch any time he wants, as long as the pitcher is not in position and ready to pitch. So it doesn't matter whether there is a change in pitchers, whether the batter has one strike or two strikes, one ball, two balls, three balls.
The rule is the same at all levels, including Little League.
Anyone who says otherwise doesn't know what he's talking about.

In little league, I dabbled with switch hitting. In little league, you can switch sides if you don't have two strikes. Sometimes I would bat left handed until I got a strike, then I would switch back to right handed. I'm not sure on the major league rules. Also, if a new pitcher enters the game, I would assume you can switch sides. Otherwise you would probably see more teams making these pitching changes late in the game.

Yes, a switch hitter (or any hitter for that fact) is able to switch sides once per pitch (if the player wants to)

a switch hitter can hit from either side of the plate, but i think he cant switch in the middle of an at-bat
