1. Nobody knows but it is said that it's either Uchiha Madara or the Yondaime Hokage.
2. Nobody knows but it is said that it's Inuzuka Rin.
3. Tobi, some say he is Uchiha Obito. And since he got caught under a boulder it's said that Zetsu found him and made him better and what he's hiding under his mask could be his disfigured face.
