My vote goes to the little known film called "Dead Alive".It's a comedy/horror movie filmed in New Zea Land,I think.The plot of the movie goes like this:A zoologist goes to this island to capture this monkey.As he captures it,he gets bitten by it and the guides freak out.They start chopping off the parts of his body that have been bitten,eventually chopping off his head.This all happens in the first 10 min!It just gets worse for there on out too.The monkey ends up at this zoo where this dorky guys Mom gets bit by the monkey.She stomps the monkey's brains out(literally)and he takes her home.As it turns out the reason the guides were chopping off parts of the zoologist was a good one,because the bite of this monkey has a bad side effect:It turns you into a zombie!I won't give away the whole story here,because you should really check this one out for yourself.But I will tell you why I think this is the most violent movie I've ever seen.It's because at the climactic ending battle of this movie,the dorky guy straps on a lawnmower and walks through the zombies,sending body parts and various goo every where.You have to watch this movie,it's simply great!

During 'Herbie Goes To Monte Carlo', the movie kept stopping at the Sunset Drive-In and this caused everyone to honk their horns. Then to throw paper cups & stuff! Then to shouting. Yes. Shouting! It got pretty violent out there that night. At least for a little while. Until the movie came back on. And everyone hid back in their cars like mice going back to their hole in the wall. And making all this crazy noise! Man, those mice sure get violent too, sometimes! ;-)
But, seriously.
Definately 'The Passion Of The Christ'.

Yes, "Dead Alive" is the all-time gore champion, but it's a gross-out film with little to no hint of realism--not that that's a bad thing, just that realistic violence has much of an effect like in "Old Boy". By the by, no one pointed out that "Dead Alive" is a Peter Jackson film...wierd.
My vote also goes to the Faces of Death films. Those are disturbing. You can't get anymore real than... real. You have to watch out for the newer ones though, some of those clips could have been seen on the local news--not too exciting. In case you haven't seen them, "Faces of Death" films are real footage of the most awful accidents, suicides, war that you can get. Similiar to what you'd find on rotten.com but in video.

Saving Private Ryan. The first 30 minutes were brutal. Also, when the medic dies, it was horrifying.
I excluded fiction films (like Saw) because they are not as gruesome as the reality of war. I did not see The Passion of the Christ, so I cannot comment on that film.

I opted not to see The Passion because of that scene. So, I'll say that his next film...Apocalypto. had more than enough violence. If I hadn't see this film, I would have picked 300.

I'd probably have to go with Hostel. I couldn't even sit through that movie. It was too much for me. But, I haven't seen Passion of the Christ, so I can't necessarily disagree with you on that one.

I would say Saw 3. All the different devices Jigsaw puts those people in. And I can think of two more: Hostel--you can do whatever you want to anybody. And Wolf Creek---when she gets her spine ripped out and cant move but is still alive.

Saw 3..I don't watch war movies.
Oh yes, the Passion was not only violent, but emotionally sad.
