One should consider only four needs for this purpose
Primary needs,Security needs,social needs, & esteem needs.
When It comes to survey of workers in India One can consider up to social needs.if Managers /executives are to be surveyed one may consider esteem needs along with other previous needs.
one should not consider self fulfillment needs (this need is very rare to track) E.g There is only one Bethovan, Da Vinci,M.K.Gandhi,Michale Angelo,etc;
Model questions (better frame your own)
Ask status of employee/worker like -Casual, Permanent,Stop gap, daily wage worker,temporary for a period.
For executives verify any contract period/Permanent position
Check up nature of the organisation -Governemnt/Private /Pub Ltd/pvt Ltd/ Non profit firm etc
1 What percentage of salary/wages you are spending for your family's food, clothing & shelter? Specify (Say 30%-50%-70%-90% (Primary needs)
2.Whether your earning is sufficient to save a portion?
3 Do you feel secured in your employment ? Y/N
4 Would you like to be popular amongst your collegues & in your social circle? If yes what are you doing for this?Are you a member of any social organisation in your society?Are you a representative of your collegues in your work place?(social need)
5 Whether for your performance if any you are rewarded by employer? like promoton, hike in pay, incentives.(social need)
6 Have you ever felt that you should be recognised in the entire industry (& in your company/working organisation) as a creative & contribution made worker? (For esteem needs)
Check self motivating level (Excellent ,Good ,Poor, dormant)
one may Ask questions about superiors leadership quality & motivating ability.E.g An inspiring manager turns the morale & motivation of subordinate to positive side of progress & growth & vice versa
Check the organisational environment whether it is motivating /dull &morose.Check also team working is present /not.
