Most high-level (and enchanted) items are based on what level YOU are at!
Try creating a new character, and going into a fort/ruin/cave - and then loading up your old character and going to the same location. Not only will the monsters be different (Skeletons with Iron longswords will be replaced Skeleton Heros with Glass longblades), but any human characters will be wearing better armor too (bandits start off in fur/leather, but get elven cuirasses at around level 15-20)
