Four is the record. There has never been 5 except it has happened before in the minors. And guy before me: Yes, you can strike out four or more batters in an inning with only 3 outs. EDIT: I see you changed your answer!
If a batter swings at strike three with 2 outs and the catcher does not field the ball cleanly and fails to tag the batter which results in the batter reaching first, then the inning continues.
He can then strike out the next batter for a fourth strikeout. Or the same as above can happen again but I don't think that has ever happened. it would be pretty embarrassing for two runners to reach 1B like that.

Yes, the record for most strikeouts in a single inning, as many have pointed out, is 4. It has been accomplished 39 different times in Major League History. The last pitcher to record 4 K's in one inning was Brad Penny for L.A. last season, coincidentally 3 days after the Dodgers batters hit 4 consecutive HRs to tie a game against the Padres in the bottom of the 9th, and eventually win on a Nomar Garciaparra walk-off HR.
Chuck Finley holds the record for striking out 4 batters in a single inning on 3 different ocassions. A brief explanation of this occurence is that on the third strike of an at-bat, there is a "hot plate" and the batter has the opportunity to advance to 1st base if the pitch is not cleanly received by the catcher.
BTW, The Saint, the "hot plate" rule is live with any amount of outs in an inning, not just with 2 outs.

John Andre was the first pitcher to record 4 strikeouts in a half inning on October 4, 1888. Before 1956 it only happened 5 times, but in 1999 alone it happened 6 times. I could name all of the pitchers, but that answer would be too long. I will give you a link for reference.

You can strike out 4 or more in an inning, actually. If a catcher drops the ball on a strike 3 pitch, the batter can run to first. 99% of the time the catcher retrieves the ball quickly and tags the batter or throws to first. But, occasionally it does happen where the batter reaches base safely.

If your just talking about the major leagues, the record is 4 and Chuck Finley (Indians) did it 3 times.
The only way this can happen is if the catcher drops the 3rd strike, the runner can get on base with a strikeout, but not an out.

It is possible to get four strikeouts in an inning.
If you strike out one batter, but the ball gets away from the catcher on strike 3, they can run to first base, and they can be safe. You get the strikeout, but not the out. If you get all the other 3 guys on strikes, you get four strikeouts in one inning.

If you are talking just about the Majors, the answer is 4. Chuck Finley did it 3 times. It is possible to have 4 strikeouts on one inning.
