You have not given near enough information to assign fault, however I would take them back to where you purchased them. A quality store will do what is needed to make it right.

I'd say it would be your fault however don't judge the glasses the first time you wear them. you should allow two or so days.

Moreover, try to find an optician which offers computer aided glasses prescription. It is far faster and usually cheaper and more accurate.

when ever you get new new glasses you should take them to the professional who wrote the Rx to be checked. If they say they match the Rx then they screwed up.other wise ti is the lab that did the lenses,
