Here are a few ideas:
You know the part when the minister or priest says "Does anyone know why this couple should not be married? Let them speak now or forever hold their peace." It would be terrible if somebody actually spoke up then and ruined the wedding. Thankfully, I've never actually seen that happen in real life.
Another thing that could ruin a wedding, from the bride's point of view, would be if somebody spilled something on her wedding dress.
I was at a wedding once where the groomsmen wrote "HELP" & "ME" on the bottoms of the groom's shoes. He did not know it was there. Everyone in the church saw it when they kneeled down together at the altar. That was more funny than terrible though.

I'd have to say killing the bride or groom would be by far the worst thing to do at a wedding!

Stick a personal part in the wedding cake while arm-pit farting here comes the bride, while vomiting on the officiator (priest, judge, whomever), apologizing the whole time by telling everyone you just smoked an over-priced eightball o' crank

Give the bride a big ol' slap on the butt for good luck like the baseball players do. I got kicked out of quite a few weddings that way.

The part that says "Anyone here object to this couple getting married" and you start screaming and running towards the front saying "YES! I object!! Our crazy sex affair is too good to let go. Please don't marry him... Marry ME INSTEAD!!" Then you grab the ring out of the grooms hand and put it on the brides hand!!
Well that's if your a guy anyways!
