Bloated government, bureaucratic obstacles, and lack of communication between agencies is what happened.
Anthrax is in Syria, some may still be buried in the desert.
War because Saddam needed to be ousted from power and he would go no other way. Saddam funded terrorists. I'd rather fight the bad guys over there than on U.S. soil.

Lots of miscommunications, inability to consolidate data between different federal and local law enforcement agencies, complacency, no effective response to terror activities prior to 9/11, etc...etc.......
