The unofficial rule is that anyone on the field in a MLB game must be in uniform. That is why umpires have uniforms, bat boys in uniforms, etc.
MLB is built on tradition, and changing something like that would upset alot of purists.

They don't have to wear a uniform. They can wear a suit and tie, but then they couldn't go onto the field to make a pitching change, argue a call, etc.. And college baseball coaches wear do wear uniforms just like the players.

Interesting question - worth a star.
I think it should be left up to individual managers. It would be neat to see a manager wear a suit like Connie Mack. I may allow sporty casual but I would have to see it.
The NFL is allowing some coaches to wear suits this year.
I was a basketball coach so that is what I was used to - basketball coaches always seemed to be the best dressers - both men and women.

College baseball coaches ARE required to be in uniform as well, as are high school coaches who want to go onto the field.

I don't know what college game you saw, but every college game I have seen from D-III up to D-I the manager is in a uniform.
The uniform should stay.

They should have to wear the team's uniform or a classy suit. Remember NFL coach Jack Del Rio wearing a suit on the Jaguar sideline? That looked sharp. However, we must never let them wear suits like the one Joakim Noah wore to the recent NBA draft. You have got to be kidding me!

I think that it's better if they wear the uniform. It makes them feel like one of the team.
