It is two different tournaments. The Confederations Cup consists of the winners of each of the FIFA confederation's tournaments. In the case of CONCACAF (North and Central America and the Caribbean) this competition is called: "The Gold Cup" and it just concluded with a U.S. victory. Go U.S.A! The South American championship is currently underway. Also, some teams are invited to play in the Confederations Cup.
The World Cup consist of teams which qualify for the competition. Each of the FIFA confederations is given a set number of qualifying teams. In 2002, for example, the three best teams from CONCACAF qualified for the world cup. Also, the host country automatically qualifies. This tournament is by far the most prestigious soccer tournament on earth and is by far the most watched sporting event - surpassing even the Olympics in world wide viewership.
The reason these two tournaments are not held in the same year is obviously because they don't want to compete with each other.

The difference is that at the confederation cup only assist the champion of each federation: conmebol, concacaf, uefa, caf ,afc and the host.
In the 2009 edition, the continental champions from South America, North and Central America and the Caribbean, Africa, Europe and Asia will join hosts South Africa in competing for the trophy.
The Confederations Cup started life as the King Fahd Cup invitational tournament which pitted hosts and organizers Saudi Arabia against three chosen opponents in 1992 and 1995. It was in 1997 that it became an official FIFA tournament and was renamed the FIFA Confederations Cup. From 2005, the scheduling of this previously biannual competition was changed to see it played once every four years and staged the year before the FIFA World Cup in the Host Country.
And at the world cup assist 31 teams plus the host, and each federation has a Preliminary Draw or qualify tournament and it's played once every four years.
And the confederations cup it's played a year before the world cup.

the FIFA Confederations Cup is a prelude to the actual FIFA World Cup. the format is the same for both tournaments but the FIFA Confederations Cup played on a smaller number of teams, currently 8 teams played in the FIFA Confederations Cup.
the FIFA Confederations Cup is held a year before the FIFA World Cup. it's a chance for the host nation to show their capability as the actual FIFA World Cup host nation so that they are well prepared for the big tournament a year after.

The Confederations Cup is a sort of mini tournament played by the winners of the major international competitions and a few invited nations from around the world. To play in the World Cup you have to either be hosting it or qualify for it- you won't ever end up being invited into it.
