write to the health department in the county you were born ( office of vital statistics) and ask them how. I wrote the hospital I was born in and they have a website you can apply for one, it cost about 15 bucks, yes I have already lost the one I got and now need to get another. it was requested for id during an inheritance issue and they never got it back to me, people lose them all the time, no big deal.

In my state Missouri we can go to the county health department and they can pull one up for you for a nominal fee. Or send request to Bureau of Vital Statics with all of the necessary information, also for a nominal fee and they will mail you a photo copy of it.

Go to the county where you were born and get a copy at the Dept. of Records. Just got mine a month ago. Prepare for a long line to wait in. (its the same place marriage and death cert. are) Some llow for an un-certified copy to be obtained online.

Every state is got a Bureau of Vital Statistic, contact the state which you were born, and they will give you a form to fill out, and tell you the fee for this certificate.

Contact your local council... if you know where you were registered you will need to go to the town hall and request it. It takes about 20 minutes and cost's about £7 for a long birth certificate (with both of your parents name on)

In Oz, we can go to the post office and fill out a form to send to Births, Deaths and Marriages.
It will cost you, but, it is an important document to have

Contact the county courthouse where you were born - usually available on the internet and find out how you have to identify yourself, etc. With identity fraud, they have become more careful and it differs from state to state.
