As Kacky pointed out... styrofoam is NOT terribly flexible.

I buy some of my styrofoam from my local hobby shop under the Woodland Scenics brand. It comes in 2'x1' sheets in thicknesses from 1/4 to 2 inches.

I've also bought SOME styrofoam at my local Home Depot... it came in 4x8 sheets 3/4 and 1 inch thick.

I also ordered some extruded polystyrene foam (pink board) through Home Depot... easy to carve.

You can't. That would be like a custom order. Home improvement centers offer sheets that are 8' long and 2 inches thick. The hard part is the 4 inches wide. Depending on how much you need; purchase a sheet and cut it 4 inches wide yourself. Use a long straight edge to assist you in the cutting. Make sure you measure the width and mark it. Use a pizza cutter(yes a pizza cutter) to scroll/mark your line to be cut first. Check the width and afterward use a long razor knife to make the final cut. There are fabric cutters shaped like pizza cutters also. That may help you also. I suggested a pizza cutter first because most people have them and would save you money on purchasing the round fabric cutter.

Styrofoam is never flexible.

Do you mean foam like they use for costumes and upholstery? If you are lucky, there may be a foam wholesaler in your neighborhood. If not, an upholstery shop could help you, or you could go thru the internet
