The earlier suggestions are good- you can also look at POPfile, which is a free, open-source filtering utility. It takes about a day of receiving e-mails to "learn" what's real and what's spam (initially it knows nothing at all, and relies upon you classifying messages in order to become smart) but after that it's pretty good. Certainly for a free product, anyway.

Onlymyemail is a great spam-filtering service at the server level, but it costs about $3 a month- cheap but not quite free.

Outlook does have it's own spam filter, but I don't think it's that good. Usually the filter is at the server level, so it would be the email server that filters the email before it reaches your client.

Outlook 2003 come with a some Anti-Spam function. Click on Action and then select the fouth option (I thing its call SPAM or undesired e-mail) ... and select option. From there your will be able to configure your anti-spam option.
