Some women are just larger there than others. Women have different size sex organs the same as a man does. If it is too loose and you and/or your partner do not get any pleasure from having sex due to it you can have surgery to tighten everthing. There is a plastic surgeon here in Phoenix who offers the "Aussie Down Under" package that gets everything a tidied back up. So, you see, it is not that uncommon. Common enough for there to be advertisements to fix it.

Child birth doesnt make it loose. If you get stitches it make you tighter.
I would say too much sex/sex toys.
