Are most destinations, like Old Montreal, in walkable distances?

Almost everything is within walking distance of downtown Montreal. The places that are a little father, can be reached by public transportation. The Montreal metro (subway) is really good. It gets you just about everywhere. And the bus does the rest. You can also bike to explore Montreal. There are bike paths across the city.

Montreal has a clean and reliable public transit system. You should not require a car while here and can do all the sites pretty much by bus, metro (subway) and walking.

The downtown core is about a ten block by four block section of town. The subway will get you to the Olympic Stadium, the old port, the casino, Six Flags La Ronde amusement park, and other sites, and the buses can get you anywhere else.

The only thing you might not be able to see is any sites OFF the island, such as the August hot air baloon show in nearby St-Jean-sur-Richieleu, or to the Laurentians for hiking, skiing (winter) or its waterslides (summer).

There are also underground tunnels that can take you all around downtown between buildings and shops. They were designed to get people out of the cold winter streets and the humid hot summer days, and will help if you get caught in a rain storm.

Old Montreal is very walkable (and not too easily drivable) from downtown and only slightly less so back (it's downhill). The downtown area is also walkable and Mount Royal and the Plateau reasonably so.

I personally wouldn't walk to the Oratory and certainly not to the Biodome, so if those are on your list, take the Metro. Little Italy and the Jean-Talon Market are also interesting but not walkable if you ask me. If you want bagels, you can get good ones in the Faubourg Ste-Catherine downtown but for my money the best are at St. Viateur Bagel. Not sure if I'd walk there even for them. Same goes for Schwartzes smoked meat.

You don't need a car though, the bus/metro are fine.

Yes and no. Most of it you can, but you will need to take the metro sometimes. It is easy to use and the cleanest I have ever seen.
Most are in walkable distances.

In Montreal every six weeks.

I've been to Montreal. It's a big city and sometimes you need to take public transportation or a car to get from point A to point B. If you're a strong walker, there's no reason not to explore on foot.
