I'll assume you mean been cheated on.
Mostly you don't know (that is why you almost lose your mind)...it's the working late and going out with friends when all he has done to date is complain about having to work so hard and just wanting to come home and relax with you. It's the unexplained missing money or in some cases lack of more money. Weight loss and better hygenie habits, constant grooming and better wardrobe. Little things will trigger a fight, where once not too long ago they never even paid attention, and the put downs directed towards YOU!

Classic signs: Coming home late repeatedly and you're not sure where he is; starts acting aloof and edgy with you; money disappearing; either a drop in sex or wants to experiment with new stuff. Then of course, whiffs of perfume on him or in car that's not yours! Someone once said if your instincts tell you he is, then he probably is, unless you are a extremely jealous person. Best way to know? Ask him/her!

2. Your spouse acting differently than before ( locking cell phone, writng emails when you are not around, making excuses to get out of the house, hours at work seem to keep changing, hiding mail )

4. seems to becoming more distant, not talking to you as much, more defensive behavior

There are always traits are things you can find. It only takes that one time for them to forget and there you will find what you're lookin for. The way they act,not makin sense,you catch them lying.

When they start to act uncomfortable and nervous around you, like they are hiding something or if you ask them If they are seeing somebody else and they show signs of lying(eyes, facial expression,etc...)

theres really no actually way to tell unless the person tells you flat out they cheated or you catch them, the only other way to find out is to follow them or keep your eye on them or flat out ask them and they may not tell the truth...
