ok the first thing to do is relax. i bet your peers are scared too. this summer try to be involved with some activities so you will get the hang of being with new people. good luck in middle school! =]

Don't sweat it. I'm going into 7th grade this year.
Only the first week is the hard part, like changing infront of people in gym, remembering locker codes, it's different from the spoiled lives in Elementary. Everyone passes rumors about how the Little 6th graders are going to get 'Trashed Canned', it neevr happens. After 1-2 weeks, everythings great.

breathe
you will quickly learn your way from one class to the next, just like you knew how to get to the cafeteria, playground, nurse's office, or music room at your other school - it's the same thing
and you won't be the only one - everyone will be a little scared, but it's really not as different as you think now
enjoy the summer! don't waste it worrying!

don't worry about it.
when you go into middle school there is always a day where you go to orientation. so when you do you most likely will get your schedule and all. then go around the school and find it during orientation so you know where everything is :) seriously middle school is a breeze; wait till HS now thats a challenge XD

Take a deep breath, relax, enjoy the next thing that comes along. Life is like this all the time. Most of what we worry about is not nearly as scary as we thing it is. In fact, most of it turns out pretty good.

Relax. Make an appt. or have your parent(s) make an appt for you to go and tour the campus before the first day of school. Have them see if the school offers orientation for incoming students to aquaint them with the campus, teachers, etc...
