Social Security Dept has a number you can call for lost cards. They'll send you a new one by mail. For your birth certificate, every state is different but usually you contact the state registrar and they will ask for legal ID and with that they will print you up a new birth certificate. There will probably be a fee anywhere from 5 to 10 bucks. Good luck.

You SO cannot just call up the social security office and have them mail you a new card. Mine burned in a fire once and the only way I could have a new one issued was by furnishing a million forms of ID including my birth certificate at the SSoffice. Even then, it took weeks to get a new one in the mail.

Birth Certificate?
Write to the capitol of the state in which you were born.
Address it to the Office of Vital Statistics, with all pertinent information, city, date of birth, parents names. There will be a fee. You may find this online.

SSI call or visit local SS office. Maybe online.
