Is this an aquatic turtle? If so see the descriptions of shell problems on thei page: http://www.austinsturtlepage.com/Care/medshell.htm

If it's a land tortoise, I suspect it's shell might be pyramiding - this is caused by a number of factors, especially too much protein in the diet, not enough humidity, and poor lighting/calcium absorption. See this link: http://www.sulcata-station.org/pyramiding.html

I'm not sure if Fish Shinanagens like me would know about turtles x_x. Maybe some people in the reptile category would know? Afterall Turtles are Reptiles.
