In baseball, a walk-off home run is a home run which ends the game. It must be a home run that gives the home team the lead in the bottom of the final inning of the game — either the ninth inning, any extra inning, or any other regularly scheduled final inning. It is called a "walk-off" home run because the teams walk off the field immediately afterward. Sportscasters also use the term "walk-off double" or other such terms if such a hit drives in the winning run to end the game. The terms walk-off hit by pitch or walk-off balk have been applied, and the latter has been dubbed a balk-off (these types of questionable walk-offs are seen by some fans as cheapening the concept). Although the concept is as old as baseball, the term itself has come into wide use only since the 1990s.

A walk-off home run is a home run hit by the home team during the bottom of the last inning (the 9th inning for the MLB). For a home run in the 9th inning to be a walk-off home run, the home team (the team batting) has to be losing, and the home run hit has to get the home team's score higher than the away team's score. After the walk-off home run, the game ends even if there are only two, one or zero outs. Basically, a walk-off home run ends the game.
Example:
The Boston Red Sox are playing at Yankee stadium against the New York Yankees. It's the bottom of the ninth, and the Yankees are losing by three. Cabrera gets walked. Jeter hits a single in which Cabrera gets to third. Matsui gets hit by a pitch. Now, the bases are loaded. A-rod's up now. The first pitch is perfect and he hits a grand slam to win the game. That is a walk-off home run. Since the Yankees are winning in the bottom of the ninth, the game is over and they all could walk off the field.
A walk-off home run does not have to be a grand slam. It could be any home run, as long as it makes the home team's score higher than the away team's.

OKay,
Let's say the Cardinals are playing the Reds, in Cincinnati. (Just an example). The Reds are losing 6-5 in the bottom of the ninth inning. They have one guy on base and they hit a homerun. The score is now 7-6, the Reds winning. The Cardinals will not get another chance to bat and come back to score therefore the Reds win and it is pointless for the Reds to keep batting and they WALK OFF the feild.

It is a game-winning homerun in the bottom of the 9th inning.
The Houston Astros have two walk-off homeruns in the past two games. The Cleveland Indians also have two walk-off homeruns in the past week.
