you need to know the new IP address www.whatismyip.com

you also need to make sure that the router has inbound port 3389 open for tcp and udp inbound traffic and it is directed to the internal ip of the machine

I use UltraVNC from http://www.uvnc.com - not only does it allow remote access and control, but it also allows file transfer both ways between the two machines. Also you need to go round to where the PC is and configure it's router and get it's external IP address. You can find out how to configure the router by going to http://www.portforward.com and looking up the make & model of router then picking the application you need to allow through.

You need to know the public IP address of the computer you're trying to remote into. (http://whatismyip.com will tell you). You need to be sure port 3389 is not blocked by any firewall at your location or theirs. If you have a router, you need to be sure port 3389 is forwarded from the router to the computers. With that, remote desktop in by using the public IP as the address of the remote computer.

Make sure the remote is turned on make sure your Protocols are checked (client for Microsoft networks,TCP/IP, and file sharing). Type ipconfig to get your ip address then type ping then the ip address to see if the nic is accessing the router.

Could be the external ip changes thats why a dns service is good like http://www.dns2go.com/ or http://www.no-ip.com/ (free)
Its easier to remember something like "www.wansremote.no-ip.com" than 65.99.127.255 too.
Also the router needs the ports open for the remote. Firewalls running on the desktop also need to be set to allow the traffic.
I might also suggest you try a VNC remote they are very nice.

Both iRemote and Teamviewer offer free remote desktop and work through firewalls/nat routers with encryption - teamviewer has certain limitations in the free version but is excellent
