The last anime episode is Ai Yori Aoshi Enishi #12, so you've already seen the ending of the anime. There is no final resolution to Kaoru's and Aoi's relationship in the anime.
The manga ended at vol. 17. Vol. 16 was just released in translated version in the USA, and vol. 17 is scheduled to be released in October.
At the end of vol. 16, Aoi is telling her parents she's abandoning her family name (just as Kaoru once did), so that she can be with Kaoru -- otherwise she'd be forced to marry the Hanabishi family's replacement for him.
I don't yet know if Kaoru and Aoi get married in the manga, but it seems likely. The cover of vol. 17 shows Aoi in a wedding outfit.
