the hospital is threatening to send me to collections even if I pay each month, but don't pay the amount they are requesting per a payment plan. I want to know if this is legal. It was my understanding that as long as you are paying something each month to the hospital, they cannot send you to collections.

In the US, if you agreed to a payment plan, that was a contract. You were expected to do what you agreed in the contract. If you didn't, you have breached the contract and they can take legal action to collect the full amount.

It is legal to tell you they will send it to collections and it is legal to actually send it to collections.

Hospitals and doctors must still follow the federal fair debt collections practices act and must follow any additional rules in your state.

You might be better off negotiating a new payment plan that you can actually live with and pay as agreed. If it does go to collections, you probably won't be able to get them to agree to payments. They might allow you make a discounted lump sum payment though, which could save you some money if are able to come up with it.

If you are not making the agreed upon payments according to the payment plan, you can be sent to a collections agency or forced into bankruptcy. Hospitals are a business and have the same rights as any other business to collect debts. You can't simply pay them $1/month and think that they can't come after you.
